All Band Ensembles:
- REQUEST FOR EXCUSED ABSENCE: See the ensemble syllabus for grading information in regard to absences from ensemble rehearsals and/or performances. Submit a request for an absence from rehearsal PRIOR to the absence. It is permissible to submit a request for an excused absence after that absence in the event of illness or family emergency. Click on the appropriate ensemble name below to access the excused absence request form.

- ALL members of UNL band ensembles must complete a contact information/photo & recording release form by the end of the second week of classes. Marching Band members must complete this form by 5:00 PM on Thursday, August 18, 2011. You'll need your Blackboard access codes to log on.
- Minors or parents of minors download the parental consent form here. Fax the completed parental consent form to 402/472-2326 or email it to band@.unl.edu. The parental consent form for minor members of the marching band must be returned to the band office by 5:00 PM on Thursday, August 18, 2011.
