Symphonic Band Excused Absence Form

See the Symphonic Band syllabus for grading information in regard to absences from rehearsals. You are expected to submit a request for an excused absence PRIOR to the absence. However, it is permissible to submit a request after the absence in the event of illness or family emergency.

Start Date Required
Enter the date when your absence will begin.
End Date Required
Enter the date when your absence will end. The start and end dates should be the same for a one day absence.
Briefly explain why you will be absent using the space above.
