Doctoral Degree Forms and Deadlines
Consult these guidelines for details as you complete the requirements summarized below.

1. Committee
- Submit completed form to: Eva Bachman, Doctoral Specialist in the Office of Graduate Studies.
- Committee composition: The committee must consist of at least four graduate faculty, including an NU graduate faculty member from outside the department or area in which the doctorate is to be granted.
- Minors: If a minor is sought, the committee should include a representative from the minor department.
- Change of Committee form: Necessary for any committee changes after the appointment form is submitted to Graduate Studies, especially changing chairs. The student's advisor must notify the Doctoral Specialist by submission of this change form.
- Appointment of Courtesy Committee Member form: Necessary only if nominating a faculty member from outside the University of Nebraska system.

2. Program of Studies, Research Requirements
Video: Preparing Your Program of Studies- The doctorate degree requires a minimum of 90 total credit hours, including a minimum of 12 hours (maximum 55) of dissertation. At the time of the filing of the Program of Studies, there should be at least 45 hours remaining to be taken, exclusive of research tools, language requirements, or collateral courses.
- Submit completed form to: Eva Bachman, Doctoral Specialist in the Office of Graduate Studies.
- Changes to the Program: After the Program of Studies has been filed with Graduate Studies, any changes should be submitted in writing. Changes to a student's Program of Studies may be submitted by the student's advisor in a memo/email to the Doctoral Specialist outlining the additions, deletions, or substitutions.

Research responsibility requirements
- Speak with your advisor regarding the need for approvals to conduct research with animals or human subjects.
- For more information: The Office of Research Responsibility provides Institutional Review Board (IRB) and Institutional Animal Care and Use Committee (IACUC) guidelines.
3. Candidacy
* And at least 7 months before final oral exam.
This form is filed when the research tools have been completed and the comprehensive examinations are passed.
- Submit completed form to: Eva Bachman, Doctoral Specialist in the Office of Graduate Studies.
- Registration requirement:Once candidacy is achieved, registration is required each semester (fall and spring) until the student graduates.
- This requirement stands even if all credit hours, including dissertation, have already been taken.
- Failure to register will result in a termination of candidacy and thus, the program.

4. Graduation Application
- Completed form due to: Graduation Services Office, 109 Canfield Administration Building, with $25 payment.
- Consequences of missed deadline: If this is not filed, a diploma will not be ordered for you and your graduation date will be postponed.
5. Oral Exam Form, Research Responsibility Followup, Incomplete Grades
* And at least 3 weeks before final oral exam.
- Before the form is submitted: (1) The two designated readers must read the dissertation and sign the form. (2) The chair or co-chairs must also sign.
- Scheduling: The final oral should be scheduled at a time when a majority of the committee, including the chair/co-chairs, can attend. The time and date of the oral examination must be recorded on the form.

- If applicable, provide proof of IRB/IACUC approval.
- All incomplete grades listed on the program of studies (except dissertation 999) must be removed with satisfactory grades to satisfy degree requirements.

1. Memorandum of Courses, Research Responsibility
Video: Preparing Your Memorandum of Courses- Completed form due to: Master's Specialist in the Office of Graduate Studies.
- Deadline notes:The Memorandum must be submitted by the student prior to the completion of half of the program. Required for candidacy.
- "Incompletes" and "no reports" count as completed courses.
- A student may not file a Memorandum of Courses and graduate in the same semester or summer session.
- Changes:After the Memorandum has been filed with Graduate Studies, any changes to the Memorandum should be submitted in writing.
- Changes may be submitted by the student's advisor in a memo/email to the Master's Specialist outlining the additions, deletions, or substitutions.
- It may or may not be possible to change the Option. Refer to the Graduate Bulletin.

- Completed form due to: Master's Specialist in the Office of Graduate Studies. Receipt of this form generates the final graduation check.
- Deadline notes: Must be filed at least four weeks (three weeks in summer) prior to the date of the oral exam or by the date shown on the left. If an oral examination is scheduled, this form must indicate the date and time of the examination and the names of the oral examining committee.
- Sections required: For all Options, Parts 1 through 4 of the form — including signatures in Part 3 but omitting signatures in Part 4 — must be completed before it is submitted to Graduate Studies. For Option I, Part 5 must also be completed at that time.
4. Preliminary Copy of Thesis
* Or at least 2 weeks before the oral exam.
- Due in the Office of Graduate Studies at least two weeks (one week in summer) prior to the oral examination date, but not later than the deadline to the left.
- The preliminary thesis may be submitted to the master's specialist electronically.
5. Incomplete Grades, Comprehensive Exam Results
- All incomplete grades listed on the Memorandum of Courses (except thesis).
Written Comprehensive Examination and/or Option II Paper Results
- Must be filed in the Office of Graduate Studies no later than the deadline listed at the left.
6. Oral Examination
- This deadline is the final day the oral examination can be held.

- Presentation of Materials to Graduate Studies for Checking/Stamping
- Final Examination Report Form
- One paper copy of abstract for Graduate Studies
- One paper copy of abstract and title page for Library

- Payment of Final Fees to Cashier/Bursar at 121 Canfield

- Return of Completed Final Examination Report Form to Graduate Studies
Ed.S. Degree Forms and Deadlines

- Committee composition:The committee must consist of at least three graduate faculty.
- Changes: If any changes to the committee are made after this form is submitted to Graduate Studies, the advisor must notify Eva Bachman in writing.

- Submit completed form (with supervisory committee chair's signature) to: Eva Bachman in the Office of Graduate Studies.
- Deadline notes:The Program of Studies form must be completed and submitted to the Graduate Studies office in the same semester or summer term of approval of the Supervisory Committee by Graduate Studies.
- The Ed.S. degree requires a minimum of 66 total credit hours. At the time of the filing of the Program of Studies, there should be at least 24 hours remaining to be taken.
- Changes to the Program: After the Program of Studies has been filed with Graduate Studies, any changes should be submitted in writing. A "Request for Change of Program" form is available from Graduate Studies, or a memo may be submitted by the student's advisor to Eva Bachman outlining the additions, deletions, or substitutions.

- Completed form due to: Graduation Services Office, 109 Canfield Administration Building, with $25 payment.
- Consequences: If this is not filed, a diploma will not be ordered for you and your graduation date will be postponed.
4. Final Report, Incomplete Grades
- Submit completed form to: Eva Bachman in the Office of Graduate Studies.
- Deadline notes: Due at least 14 days before graduation. All information and signatures should be complete at that time except the signature of the Dean of Graduate Studies.
- All incomplete grades listed on the program of studies (except dissertation 999) must be removed with satisfactory grades to satisfy degree requirements.
