Deadline Requirement
Appointment of the Supervisory Committee*
  • Submit completed form to: Eva Bachman in the Office of Graduate Studies.
  • Deadline notes: The form must be filed by the student prior to the completion of 42 credit hours.
  • Committee composition: The committee must consist of at least three graduate faculty.
    • Changes: If any changes to the committee are made after this form is submitted to Graduate Studies, the advisor must notify Eva Bachman in writing.

Program of Studies*
  • Submit completed form (with supervisory committee chair's signature) to: Eva Bachman in the Office of Graduate Studies.
  • Deadline notes: The Program of Studies form must be completed and submitted to the Graduate Studies office in the same semester or summer term of approval of the Supervisory Committee by Graduate Studies.
    • The Ed.S. degree requires a minimum of 66 total credit hours. At the time of the filing of the Program of Studies, there should be at least 24 hours remaining to be taken.
  • Changes to the Program: After the Program of Studies has been filed with Graduate Studies, any changes should be submitted in writing. A "Request for Change of Program" form is available from Graduate Studies, or a memo may be submitted by the student's advisor to Eva Bachman outlining the additions, deletions, or substitutions.

Application for Degree
  • Completed form due to: Graduation Services Office, 109 Canfield Administration Building, with $25 payment.
  • Consequences: If this is not filed, a diploma will not be ordered for you and your graduation date will be postponed.
Final Report for the Ed.S.*
  • Submit completed form to: Eva Bachman in the Office of Graduate Studies.
  • Deadline notes: Due at least 14 days before graduation. All information and signatures should be complete at that time except the signature of the Dean of Graduate Studies.
Incomplete Grades
  • All incomplete grades listed on the program of studies (except dissertation 999) must be removed with satisfactory grades to satisfy degree requirements.
