Prospective Students
Welcome to the University of Nebraska-Lincoln! The University Health Center (UHC) provides healthcare and education to UNL students. So that we may serve you better, we ask that you provide the following:
- Pre-Enrollment Health Requirement
- Medical History Form and Permission to Treat:
- Once you are enrolled at UNL, you will receive this form in the mail and we ask that you fill it out and return it to the UHC. The information provided on this form will help us provide for your individual healthcare. The information is confidential and will not be released without your permission. If you are under 19 years of age, a parent or guardian must sign the permission to treat on the reverse side.
- Meningococcal Vaccination
- Pre-Existing Health Condition:
- If you have a condition that requires regular treatment, medicines or monitoring, one of our providers in the main clinic will coordinate your care with your current provider.
- Health Insurance
- Recommended Immunizations
