These four forms can be completed online and submitted electronically. DO NOT "save" the form at any time prior to submitting it electronically. If you save the form or any part of the form before you electronically submit, it will not work properly:

The following forms should be completed on your computer and printed. These cannot be submitted electronically.
 
• F-1 Academic Advisor Extension of Stay Recommendation
• I-20 Application
• Academic Advisor Certification* J-1 Classification
• Certification for Reduced Course Load for F-1 Students (To be Completed by Academic Advisor)
• Financial Certification for International Students
• International Student/Scholar Data Form
• Medical Certification for Less-Than-Full F-1 Student Status
• Medical Certification for Less-Than-Full J-1 Student Status
• Request for J-1 Scholar/Professor Extension of Stay
• Request for J-1 Student Extension of Stay
• Request for FORM DS-2019 (For Dependents)
• Request for FORM DS-2019 (For Visiting Scholar)
• U.S law requires all non-U.S. citizens (except for holders of A or G visas) report a change of address within 10 days of moving by completing a USCIS Change of Address (Form AR-11)
• International Student Scholarship Recommendation
