Services for Students with Disabilities

Student Responsibilities


Students who need accommodations should contact the Services for Students with Disabilities office (SSD) and schedule an appointment with either the Director or the Assistant Director to complete an Accommodation Planning Form. The planning form and appropriate documentation of your disability will be forwarded to the ADA/504 Compliance Officer, who will issue an Individual Accommodation Plan. Once you have received this plan, it is your responsibility to initiate the services you need each semester .

The following outlines your responsibilities for different accommodations:


Test Taking Accommodations

  1. Attend the first week of class.
  2. Fill out the Testing Accommodation Request Form available from the SSD office or our website. The course syllabus will help to properly fill out this form.
  3. A letter will be e-mailed to your professor about your testing accommodations. Ask the test coordinator when this e-mail will be sent out.
  4. Schedule an appointment with your professor to talk about your accommodations.
  5. If you are taking exams in the SSD office you must schedule to take your exams with the SSD office at least 3 days before the exam date.
  6. If you are late for an exam or you want to change the time and/or date of your scheduled exam for any reason, you will need written approval from your professor.

Notetaker Accommodations

  1. Attend the first week of class.
  2. Fill out the Notetaker Request Form that is in the SSD office or on our website. If the professor provides notes on Blackboard, check to see if your classmates are taking additional notes before requesting a notetaker.
  3. The staff at SSD will recruit a notetaker by e-mailing members of the class or going to class to recruit a volunteer. If you know someone in class and feel comfortable, you can ask her/him to take notes. They may use the SSD office to copy their notes for you. Please inform the SSD staff if you choose to obtain a notetaker yourself.
  4. If you need notes immediately, due to tests or quizzes, identify this on the form.
  5. It is your responsibility to complete the Notetaking Feedback Form if there are problems with the notes for your class.
  6. If you drop a course, please notify the SSD office so we can cancel the notetaker.

Alternative Materials Format Accommodation

  1. Complete the Taped Text or E-Text Request Form available at the SSD office.
  2. This form should be filled out after you have completed priority registration for the next session. This insures adequate time to get the materials in alternate format.
  3. If the request is late, you are required to submit a copy of the syllabus for each class.
  4. All alternative materials need to be returned to the SSD office at the end of the semester.

Sign Language Interpreting or Real Time Captioning Services

  1. Discuss with the Interpreter/Captionist Coordinator what services you will need for each class.
  2. This request should be made after you have completed priority registration. We request at least two weeks notice.
  3. It is your responsibility to contact the Coordinator if you are unable to attend a class or event.
  4. If you drop a class, you need to notify the Coordinator immediately.
  5. Failure to attend class regularly and notify the Coordinator will result in corrective action.

General Information

  1. Accommodation plans can be revised. If you find you need additional services, schedule an appointment with the Director of SSD to request a revision.
  2. UNL has a grievance procedure. If you believe you have been discriminated against on the basis of a disability, you will need to contact the Director of Services for Students with Disabilities.
