ONLINE UCARE APPLICATION FORM PROCESS
- The student completes their portion of the online application, providing the name of their faculty sponsor. The student will receive a confirmation email.
- Once the student submits his/her portion of the application, an email is sent to the faculty sponsor with a web link to complete the faculty portion of the application.
Before you submit your UCARE application:
- Make sure that all fields are complete and accurate before submission.
- Make sure all words are spelled correctly. This application represents you and your faculty sponsor. Make sure it reflects well on both of you.
UCARE YEAR 1 Application (for students new to the program)
Students who are applying for only one year of UCARE - please fill out a UCARE Application Year 1 form.
Student section:
- Your first and last name
- Your email address -- please be sure to give an address that accepts bulk mail or know how to access a bulk mailing.
- Telephone number
- Your student ID number - located on your NCARD (not your social security number).
- Your major
- Your current year in school (not based on credit hours, but based on the number of years left until graduation).
- Your GPA (estimated)
- Your expected date of graduation
- The title of the project on which you will conduct research
- Summary Statement. Provide a summary of the project on which you will be working, including your proposed role in this project (no more than 500 words). Please include the scientific or creative methods to be employed and the significance of the work to the field of knowledge in general. The student and faculty sponsor may work together on writing the summary statement.
- The duration of the project. The entire span of a UCARE "year" is June 1, 201x to July 31, 201y. However, we do not expect that students would request that entire time period. For example, you may work on the project from June 1 through August 31, or from late August until the beginning of May. You may plan to only work on a project during the spring semester, or perhaps you want to extend your work from the summer throughout the academic year.
- Funds requested. Please estimate how much UCARE funding you will need based on $8/hour, working about 10 hours/week for the duration of your project. The maximum award for the first year is $2000.
- Type the name of your faculty sponsor in the box and click on the "search faculty" button. It is better to not use any title, such as Dr. or Professor. Just first and last name.

Faculty section:
- Please complete the contact information and include your campus ZIP. Please note that visiting faculty are not eligible for UCARE.
- Please state the role that the student will play in your research or creative activity. You might want to detail the kinds of tasks the student will undertake.
- Please indicate whether the YEAR 1 project will lead to a YEAR 2, more independent research project. If not, please state why.
- Please comment on the ability of the student to carry out this research. How well do you know the student? Have you worked with this student before?

*Year 2 students must fill out the Year 2 application to re-apply for a second year of UCARE funding.
