Agreement Forms (for students awarded UCARE funds for 2008-09)
Once you have received the UCARE award letter for the 2008-09 academic year, please complete this form to accept your UCARE award:
For Year 1

For Year 2
Award letters are sent every year to Year 1 and Year 2 UCARE students that have been accepted into the UCARE Program.
Please return the Agreement Form by April 28, 2008 to the Office of Undergraduate Studies (201 Seaton Hall 0683).
Participant Information Form (for Year 1 and Year 2)
UCARE students accepted into the UCARE Program for 2008-09 - please fill out this short form by April 28, 2008 notifying us of when you want to begin your UCARE project. Funds for the summer will be available (at the earliest) June 1, 2008.
