Agreement Forms (for students awarded UCARE funds for 2009-10)
Once you have received the UCARE award letter for the 2009-10 academic year, please complete this form to accept your UCARE award:
For Year 1
OR
For Year 2
Award letters are sent every year to Year 1 and Year 2 UCARE students that have been accepted into the UCARE Program.
Please return the Agreement Form by April 27, 2009 to the Office of Undergraduate Studies (201 Seaton Hall 0683).
