Campus Bands Absence Request Form

See the Campus Band syllabus for grading information in regard to absences from rehearsals and/or performances. You are expected to submit a request for an excused absence PRIOR to the absence. However, it is permissible to submit a request after the absence in the event of illness or family emergency

Please provide accurate, but brief, information regarding your proposed absence.

Start Date Required
Enter the date when your absence will begin.
End Date Required
Enter the date your absence will end. The start and end date should be the same for a one day absence.
Please briefly explain why you will be absent.
