Whether you seek an informational table, an educational workshop, or a speaker, this form will help us to discern the best way to serve your needs. The form has three parts: first, general information about the unit requesting an engagement, second (if appropriate), information about the larger event the engagement might be a part of and third, specific details regarding the nature of your educational, table, or speaker request. As a note, we typically require 4 weeks' notice to fulfill any request.
